STUDENTS: Please include proof of student status (e.g., photocopy of valid student ID card)

Instructions for Internationally-based registrants: Please print out, complete, and send in this form only to the address below postmarked no later than August 31, 2001, to enjoy pre-registration rates. You (internationally-based registrants only) may pay total amount in cash ($US) upon your arrival at the conference. STUDENTS: Please bring proof of student status (e.g., valid student ID card)

*Note about lunches: As there is no cafeteria at the conference center, we highly recommend that you take advantage of the conference lunch option coordinated for your convenience: pre-ordered local-style boxed lunches will be served on the garden level of the conference center on Friday (10/5) and Saturday (10/6).
